Advance Care & After Death Care Directives

Hand holding a dandelion with the text "my final wishes"

What are your final wishes?

Death is a journey that all must take, and establishing your path before the time arrives can make that journey easier for you and your loved ones. Documenting and communicating your final wishes is critical so that your loved ones know what you want and what to do when the time comes.

Perhaps you have thought about starting, completing, reviewing, or even finding your end-of-life documents. This workshop is unique in that we discuss After-Death Care Directives (your desired wishes for the care of your body after death) in addition to Advance Care Directives, which address your care leading up to your death. We create an environment where you can think about end-of-life care more deeply and offer you an opportunity to participate in a casual group setting where you will work to complete your advance care and after-death care documents. The goal: a notarized set of end-of-life documents and knowing what your next steps are.

Topics & Documents Covered
  • Living Will
  • Health Care Power of Attorney
  • 5-Wishes Document
  • Declaration of Disposition of Physical Body After Death
  • Death Certificate
  • MOST Document
  • Cremation Authorization
  • Final Arrangements
  • Options for After-Death Care: Home Funeral/Burial/Cremation
  • Laws and Regulations Regarding Disposition of Our Body

This course provides a strong foundation for the Home Funeral and Death Care Midwife Training and is a prerequisite for the Preparing for Your Own Good Death and Life course.

Testimonials

  • Yesterday, Jane’s body lay in state at Ruth’s house. No more Roman Senator. Jane looked like a Buddha. Intensity of focus suggested she was learning to walk or fly or swim the channels of light, while the vigil of the living continued around her.

  • What we did was the difference, for my family, in incurring debt and remaining solvent, and in saying an intimate goodbye in a language we all understood, as opposed to saying goodbye in the mortuary chapel of kindly paid professionals.
